This is especially complicated for patients with multiple chronic conditions, who can be treated by different specialists. The advent of electronic health records granted doctors the opportunity to have all the available information at hand – on the computers at their workstations. However, the challenge of representing these data in an easy-to understand way remained. The Medi_Vizz application allows doctors to visualise individual patients' diagnoses and laboratory analysis data. Medi_Vizz has a user-friendly interface and creates comprehensible plots with medical data.
