Preclinical studies show that polysaccharides, triterpenoids and polyphenols from Inonotus obliquus (Chaga) activate PI3K/Akt-GLUT4 translocation, phosphorylate AMPKα, enhance ubiquitin–proteasome throughput and remodel intestinal microbial networks—mechanisms predicted to restore metabolic homeostasis and proteostasis. On this basis, a double-blind, randomised, placebocontrolled trial in 200 adults (≥ 40 y) with T2DM will assess whether 12 weeks of 500 mg day-1 standardised Chaga extract adjunctive to usual care lowers glycated haemoglobin and improves HOMA-IR (primary endpoints), cognitive composite z-scores, microbial α/β-diversity and cardiometabolic biomarkers (secondary endpoints). Power analysis (85 %, α = 0.025) indicates sensitivity to a ≥ 0.5 % absolute HbA1c reduction using baseline-adjusted ANCOVA with complementary mixed models. Because dissertation-timeline constraints preclude immediate enrolment, trial registration and ethics submission remain forthcoming, consequently, this thesis provides a complete protocol, feasibility assessment and simulation-based power analysis, but no efficacy or safety data. The participants were from Internal Medicine Clinics (n=17) and Anaesthesiology and Intensive Care Clinics (n=14) at Tartu University Hospital, who completed a web-based, self-administered and open-ended questionnaire during January 2025. The data was analysed using a descriptive qualitative thematic analysis inductive approach. Three key themes were identified: nutritional screening and assessment practice, collaboration with the nutrition support team, and roles and perspectives of health professionals in nutritional care. The study findings provide insights into hospital nutritional care by identifying areas for improvement. This study concluded the need to increase the awareness of the nutrition support team among hospital staff and to implement routine nutritional screening aligned with international guidelines. It is associated with higher levels of complications and mortality, poorer outcomes and higher hospital costs. Thanks to various initiatives in the field of nutrition, new screening and assessment methods for the detection of malnutrition are constantly being developed, which are intended to be time-saving and convenient. However, there is evidence of them being not sufficiently implemented in medical practice. With our pilot study, we hope to develop a new idea to address this problem. The idea of automating nutritional screening seems attractive and could help healthcare facilities around the world not to underestimate the importance of diagnosing malnutrition. By analysing routinely done blood tests on patients, the results of their nutritional screening and relying on literature data, we have developed a pilot formula that allows us to estimate the risk of malnutrition in a patient. While our results do not yet report the formula results being as sensitive as existing screening or assessment methods, however, it is a promising and prospective direction for the effective diagnosis of malnutrition. The correlation of malnutrition not only with blood parameters, but also with patient age, was an area of interest to me. Existing studies indicate a more relevant biological age for predicting morbidity and mortality, which differs from the chronological age due to changes at the cellular and molecular level. Assuming that this indicator correlates better with the presence of malnutrition, in the second part of the thesis an analysis of this relationship was performed.
