Abstrakt
Bakalaureusetöö eesmärk on uurida, kuidas sõltuvad elukestusanalüüsi järeldused kasutatavast meetodist olukordades, kus ravimõju muutub ajas. Töös võrreldakse logaritmilise
astaktesti ja tõkestatud keskmise ellujäämisaja meetodi käitumist simulatsiooniuuringu abil kolmes stsenaariumis: proportsionaalsed riskid, hilinenud ravimõju ja ristuvad
ellujäämiskõverad. Lisaks rakendatakse mõlemat meetodit USA Veteranide Administratsiooni kopsuvähiuuringu andmetel. Simulatsioonid näitasid, et logaritmiline astaktest on
kõige võimsam proportsionaalsete riskide korral, kuid selle võimsus langeb oluliselt mitteproportsionaalsete riskide korral. Tõkestatud keskmise ellujäämisaja meetod võimaldab
kirjeldada ravimõju suurust ja ajalist struktuuri ka olukordades, kus proportsionaalsete riskide eeldus ei kehti, eelkõige ristuvate ellujäämiskõverate korral. Kopsuvähiandmestiku
analüüs kinnitas, et kaks meetodit täiendavad teineteist ning nende kooskasutamine annab ellujäämisanalüüsist sisukama ülevaate.
The aim of this bachelor’s thesis is to investigate how conclusions from survival analysis depend on the choice of method in situations where the treatment effect changes over time. The log-rank test and the restricted mean survival time method are compared using a simulation study across three scenarios: proportional hazards, delayed treatment effect, and crossing survival curves. Both methods are also applied to data from the US Veterans Administration lung cancer trial. The simulations showed that the log-rank test performs best under proportional hazards, but its power drops notably when this assumption is violated. The restricted mean survival time method allows quantifying the magnitude and temporal structure of the treatment effect even in situations where the proportional hazards assumption does not hold, particularly when survival curves cross. The analysis of the lung cancer dataset confirmed that the two methods complement each other and that their combined use provides a more complete picture of survival differences.
